How to access previously submitted forms

    in Student Health Tracker

    You recently rejected a form and are waiting for the parent or student to submit a new one. In the meantime, there may be relevant information on the previously submitted form (“historical document”) that you need to access. You can view/download any historical document submitted to a student’s account.

    • Step-by-Step Instructions
    Go to Front Desk.

    Go to Student Health Tracker.

    Enter the name of the student into the Search by Student field.

    Select preferred student and click Search.

    Find the name of the Requirement and then locate the Actions column to the far right of that requirement.

    Click the Cog icon associated with the required form.

    Select View History.

    You will be able to view and download any form uploaded to the student’s account, no matter if you approved it five years ago, rejected it, or never viewed it at all.

    Combining documents
    If you need to combine a document from the History with a current upload you will need to first download the historical document and save it to your computer. You can then use the multi-upload tool to browse for that document and combine it with a current form.
